If you are a licensee, certificant, or registrant, and you would like written verification of your license, certification, or registration sent to a third party (e.g. another state) , this is what you must do.

     Send your request to the Office of Professional Regulation, Attn: (Specific profession), National Life Bldg., North FL2, Montpelier, Vermont 05620-3402, and include the following:

       *Your written request, and
       *a check in the amount of $20.00 (payable to Secretary of State), and
       *include any forms to be completed (if applicable)
