Verification of Licenses, Certifications, Registrations
I. Verification of your license, certification, registration
II. To Verify that someone is a licensee, certificant, registrationIf you are a licensee, certificant, or registrant, and you would like written verification of your license, certification, or registration sent to a third party (e.g. another state) , this is what you must do.
Send your request to the Office of Professional Regulation, Attn: (Specify profession), 26 Terrace Street, Drawer 09, Montpelier, Vermont 05609-1106, and include the following:
*Your written request, and
*a check in the amount of $20.00 (payable to Secretary of State), and
*include any forms to be completed (if applicable)
If you would like to verify whether someone is a licensee, certificant, or registrant go to our DATA BASE to search. If you would like us to send you written verification follow the instructions in Paragraph I.
