If we have received your application or any documents for your file after February 9, 2009, your application status can be found here. You will be notified via email when a change in your status (approval or deficiency) has occurred. Otherwise, click here to check on your application.
If you are represented by an Agency, contact them for information PRIOR to contacting the State of Vermont. All information concerning your application file will first be conveyed to your agency if you have chosen to be represented during the application process. Agency representation is not required to apply to the State of Vermont.

International Nurse NCLEX Pass Endorsement Application This application is for Internationally educated nurses who have passed the NCLEX through a different state, but who do not yet hold a US license.
Note: As of July 1, 2009, all incoming exam and NCLEX pass endorsement applications must be these versions of the application (Uploaded July 1, 2009). No earlier forms will be accepted. If any arrive in this office they will be mailed back to the sender.
Document Forwarding Request Form This form is for applicants who will be sending an application after the July 1, 2009 changes. If you have already sent official school or licensing documents to the State of Vermont and have not yet applied but will now be obtaining a Credentials Evaluation Service (CES) Report, you may request that Vermont forward your official documents to one of the credentialing agencies we work with using this form.

NCLEX Re-Take Forms and Information
NCLEX-RN Re-Take Application As of July 1, 2009 this version of the re-take application will be the only version accepted. NCLEX-RN Re-Take applications are processed within 45 days of receipt.
