Q. How do I change my Address?
A. Fill out the following form and send it to this office through postal mail. Address Change Request Form
Q. How long is the application process?
A. Initially, applications and renewals are reviewed by clerical staff to verify that ALL required documents have been received. When complete, the application is sent to an RN staff member for review of the transcript and practice guidelines. The process may take up to 30 DAYS from the receipt of all materials.
Q. Who is responsible for submitting practice guidelines?
A. The applicant - NOT the medical staff office or staffing agency - is responsible for assuring that the application, including practice guidelines, are complete and contain all the required elements as stated in the Administrative Rules on the Home page for Nursing on this site.
